Remote Cardiology Patient Navigator (RN/MA/CNA)

About the role

This remote position supports a busy US‑based cardiology team by guiding patients through their care journey. The role focuses on communication, scheduling, documentation, and coordination of remote monitoring for a high‑volume caseload.

Key responsibilities

• Manage patient scheduling, reminders, and follow‑up workflows within EMR/EHR platforms.
• Conduct telephonic outreach to reinforce care instructions and coordinate next steps.
• Assist patients navigating cardiology pathways, including CHF management protocols.
• Track testing schedules, appointments, and remote monitoring data for 150+ patients.
• Escalate clinical concerns to providers while adhering to scope (no diagnosing or prescribing).
• Maintain accurate clinical documentation and ensure HIPAA‑compliant records.
• Support billing documentation and insurance coordination as needed.

Required profile

• Clinical credential (CNA, MA, RN or equivalent).
• 3+ years experience in healthcare, telehealth, or medical office settings.
• Proven experience in care coordination or management, preferably remote or telephonic.
• Ability to handle a high‑volume, fast‑paced environment with strong patient‑first attitude.

Required skills

• Proficiency with EMR/EHR systems.
• Experience using remote patient monitoring platforms.
