Remote Cardiology Patient Navigator (RN/MA/CNA)

About the role

Our client is looking for a Remote Patient Care Coordinator, also known as a Cardiology Patient Navigator, to support a busy US‑based cardiology team. The position is fully remote and focuses on patient communication, scheduling, care coordination, and clinical documentation for a high‑volume caseload.

Key responsibilities

• Manage patient scheduling, reminders, and follow‑up workflows in EMR/EHR systems.
• Conduct telephonic outreach to reinforce care instructions and coordinate next steps.
• Support patients navigating cardiology pathways, including CHF management protocols.
• Track testing schedules, follow‑up appointments, and remote patient monitoring workflows.
• Coordinate with the clinical team to ensure continuity of care for 150+ patients.
• Escalate clinical concerns to providers within scope (no diagnosing or prescribing).
• Maintain accurate clinical documentation and ensure HIPAA compliance.
• Assist with billing documentation and insurance coordination as needed.

Required profile

• Clinical healthcare background (CNA, MA, RN, or equivalent).
• 3+ years of experience in healthcare, telehealth, or medical office settings.
• Strong communication skills and comfort managing high‑volume phone interactions.
• Ability to work independently in a remote environment and adapt to evolving processes.

Required skills

• Proficiency with EMR/EHR systems.
• Experience using remote patient monitoring platforms.
