Remote Cardiology Patient Navigator (RN/MA/CNA)

About the role

Our client is seeking a Remote Cardiology Patient Navigator (also called Remote Patient Care Coordinator) to support a busy US‑based cardiology team. Working from anywhere, you will handle patient communication, scheduling, care coordination and clinical documentation for a high‑volume caseload of 150+ patients.

Key responsibilities

• Manage patient scheduling, reminders and follow‑up workflows in EMR/EHR systems.
• Conduct telephonic outreach to reinforce care instructions, support CHF management protocols and guide patients through cardiology pathways.
• Track testing schedules, remote monitoring data and coordinate follow‑up appointments.
• Collaborate with the clinical team to ensure continuity of care and escalate clinical concerns within scope.
• Maintain accurate documentation, ensure HIPAA compliance and assist with billing and insurance coordination.

Required profile

• Clinical credential such as CNA, MA, RN or equivalent.
• Minimum 3 years experience in healthcare, telehealth or medical office settings.
• Proven experience in care coordination or management, preferably in a remote/telephonic environment.
• Ability to handle a high‑volume, remote workload with strong organizational skills.

Required skills

• Proficiency with EMR/EHR systems.
• Experience using remote patient monitoring platforms.
• Knowledge of HIPAA compliance standards.
