Cardiology Patient Navigator – Remote

About the role
The Cardiology Patient Navigator supports a busy U.S.-based cardiology team by managing patient communication, scheduling, and care coordination. Working remotely, you will ensure patients follow their care plans, especially for heart‑failure management, while maintaining accurate clinical documentation.
Key responsibilities
- Manage patient scheduling, reminders, and follow‑up workflows in EMR/EHR systems.
- Conduct telephonic outreach to reinforce care instructions and coordinate next steps.
- Track testing schedules, appointments, and remote monitoring workflows for a caseload of 150+ patients.
- Escalate clinical concerns to providers within scope (no diagnosing or prescribing).
- Maintain HIPAA‑compliant documentation and support billing and insurance coordination.
Required profile
- Clinical healthcare background (CNA, MA, RN, or equivalent).
- 3+ years in healthcare, telehealth, or medical office settings.
- Experience in care coordination or remote patient management.
- Bilingual proficiency (B2+ English and B2+ Spanish).
