About the role
The Cardiology Patient Navigator supports a busy U.S.-based cardiology team by managing patient communication, scheduling, and care coordination. Working remotely during U.S. Central business hours, the Navigator ensures patients stay connected to their care plans and clinical instructions.
Key responsibilities
- Manage patient scheduling, reminders, and follow‑up workflows in EMR/EHR systems.
- Conduct telephonic or remote outreach to reinforce care instructions and coordinate next steps.
- Support patients navigating cardiology pathways, including CHF management protocols.
- Track testing schedules, appointments, and remote monitoring workflows for a caseload of 150+ patients.
- Coordinate with the clinical team to maintain continuity of care and escalate clinical concerns as needed.
- Maintain accurate clinical documentation, ensure HIPAA compliance, and assist with billing and insurance coordination.
Required profile
- Clinical healthcare background (CNA, MA, RN, or equivalent).
- 3+ years of experience in healthcare, telehealth, or medical office settings.
- Experience in care coordination or management, preferably in remote/telephonic environments.
- Bilingual proficiency in English and Spanish (B2+ level required).
- Ability to handle a high‑volume, remote workload with strong patient‑first focus.
Required skills
- Proficiency with EMR/EHR systems.
- Experience using remote patient monitoring platforms.
